How to Bill RTM Codes from Past Plans of Care

Providers have the ability to review and bill previously unbilled & billed RTM codes from past plans of care. This feature ensures the retention of RTM history and provides access to historical RTM data even after a patient has been discharged.


When an RTM patient with both unbilled and billed codes is discharged and later re-admitted, a 'Show Past Plans of Care' button will appear under the 'Billing' tab in the RTM Details Tray.

Step-by-step guide:


1. Click on a discharged patient's card to open their profile. 

2. Once in the patient's profile, click on the 3 vertical dots next to the message icon to open the details menu, then click on 'Readmit Patient'


3. A notification pop-up will appear, alerting you that the patient will now have access to their Home Exercise Program and that you will be able to view previous plans of care. Click the 'Readmit' button.

4. A notification message "Success! Patient has been readmitted. New RTM enrollment date and billing period has started." letting the provider know that the patient has been successfully readmitted.

5. Click on the 'RTM' label to open the Patient RTM Details tray.

6. Navigate to the Billing tab, click the 'Show Past Plans of Care' button to view past plans of care.

7. Once you click on 'Past Plans of Care' previously billed and unbilled RTM codes will be displayed on the list, allowing you to see which codes were billed or can be billed for past plans of care.

8. To revert back to the current plan of care, simply click on the 'Back to Current Plan of Care' button.
