Creating a Care Plan (PT University)
What is PT University?
PT University is a seven-week educational video series in which 9 videos are dripped to the patient over a period of 37 days. In addition, the videos are dripped according to how the patient is progressing.
Videos support your patient education and communication throughout care and prevents patient drop out. Each video series is diagnosis-specific, set by the Physical Therapist and easily incorporated into existing workflows.
For RTM billing clinics, the videos are dripped to the patient over several weeks which means in addition to the patients doing their exercises on the app, messaging the provider, tracking their steps; now they’ll be receiving these educational videos (another opportunity for you to get a day with the data point.) Diagnosis (Program) Sequences
We evaluated over 70 000 cases and looked at the largest categories with regards to diagnoses:

For each bullet point listed there is a full nine video, seven week series that is dripped out to the patient.
How to Create a Care Plan:
1.) Log in as an Admin or Therapist and click "+Add Patient."
2.) Fill out the information required, and if your clinic has several locations - pick the one that is relevant to your patient. Click the “Save & Add Care Plan” button.
- When "Save" is selected, the patient will be added to the system but will not receive an activation email.
A Success! Patient has been created pop-up will appear at the top right of the page once a patient has been successfully created.
3.) Once the "Save & Add Care Plan" button is selected, the Physical Therapist will be directed to the “Care Plan” page.
4.a.) From here, you can set the duration (in months,) the total sessions per episode, the re-evaluation time (in weeks) and select from the program dropdown menu a specific plan of care for your patient. The programs range from Shoulder to Lower Back.
Selecting "Other" from the program drop-down menu will not create or add a PTU Care Plan for that patient, and is used for patients you do not wish to add to the PT University.
For example, we have set Elon Musk’s Care Plan for a duration of 8 months; a total of 14 sessions; the re-evaluation for 2 weeks’ time and the program we’ve selected for her is “Shoulder OA,” his diagnosis.
All of the above metric sliders are editable however, please note that at the moment, once the Care Plan is created it cannot be edited.
b.) The NPS toggle can be enabled or disabled and is used for the NPS email survey sequence. The toggle is enabled if it is blue in color, and disabled if gray. If toggled on, the patient will receive automated surveys.
- If the toggle is disabled it means that the survey will not be sent out on an automated schedule, but providers will still have the option to "Send NPS" manually on the patient's profile.

The patient will receive an email, just like the one below, where they'll be asked to give a rating from 1 (being the lowest) to 10 (being the highest):
Patients will then be prompted to enter a review, and click "Submit."
After the patient has submitted the reason for their score, they following pop-up will be displayed:
5.) Once you’ve added the desired metrics for the patient’s care plan, click the “Next” button on the bottom right of the page.
You’ll be directed to the “HEP Creation” page.
6.) Here, the provider is able to search for and select exercises and Create a Prescription to assign to the patient.
7.) After clicking the “Assign” button at the bottom right of the screen, you’ll be directed to the patient’s profile, where you can view the Care Plan under the "PT University” section. The video series will be displayed as well as in indicator of when the patient is due to watch a video.
Deleting a Care Plan
8.) Providers also have the option of deleting the patient's Care Plan by clicking the red "Delete Care Plan" button and following the prompts.
